Care At Home
Care at home is an integral NHS service.
We help by providing medication compliance assessments, simplify regimes and medication to improve outcomes, supply administration charts and, if needed, provide weekly compliance aids (dossette boxes)
How it works
Simply contact the pharmacy if you feel you need an assessment, an administration chart or a compliance aid.
We can take referrals from the care at home teams, nursing staff, Gp Surgery.
After a request is made a team member will be in contact with the patient to arrange a quick consultation that involves a questionnaire.
If found needed, an administration chart or compliance aid will be provided every 4 weeks, a record will be sent to the GP along with any improvements or simplifications.
